Properly documenting your care
To ensure proper documentation and billing of your visit, it is imperative that the appropriate forms be completed, signed and brought to your first visit.
A packet of these forms will be sent to you in the mail after you set up your appointment. For a PDF of these forms, download below.
If you have a NextMD online secure patient portal account (for more information, see our NextMD brochure) and wish to request past medical records be made available on your account, log in here.
If you DO NOT have a NextMD Patient Portal account, you may establish one by calling 612.775.6212 or sending an email to MedicalRecords@tcspine.com.
