Properly Documenting Your Care

To ensure proper documentation and billing of your visit, it is important that the appropriate forms be completed, signed and brought to your first visit.

An email will be sent with instructions on how to sign up for Medfusion (our patient portal). From there, you will complete the Registration form and the Initial Visit form and will submit them electronically. You may download a PDF of our forms from the Download Forms box.
