Medfusion Secure Patient Portal
Medfusion is a secure, online patient portal that allows you to view your Twin Cities Spine Center (TC Spine) medical records online.

Log in to Medfusion now if you already have a Medfusion Patient Portal Account.
If you do not have a Medfusion Patient Portal Account, you may establish one by calling 612.775.6212 or sending an email to MedicalRecords2@tcspine.com.
Sharing Your Records
If you need your records shared with another medical provider, besides your primary or referring provider, you will need to complete a Release of Information Authorization Form. A patient, parent or legal guardian may complete the form to release records. An individual acting as an attorney through a power of attorney may also use this form.
To protect your privacy, the law permits disclosure of information only if the patient (or his or her legal representative) consents in writing or if the law otherwise permits it.
The form may be completed electronically, with the exception of the signature. You must print the form to sign it. After completing, signing and dating the form, please submit it to TC Spine medical records staff via one of the following:
