Initial Visits


Information pertaining to previous evaluation and treatment of spinal conditions is essential. All previous medical records must be sent to Twin Cities Spine Center two weeks prior to an initial visit or hand carried to the appointment. If these medical records and scans are not available at the time of the initial visit, the appointment may need to be rescheduled.

It is the patients responsibility to bring the following to an initial visit at Twin Cities Spine Center:

  1. Previous x-rays, CT scans or MRI scans: Actual films and reports must be provided; a report alone is insufficient.
  2. Results of previous testing or treatment (EMG's, physical therapy, etc.)
  3. Any corset, brace or support prescribed for use.
  4. Completed Initial Visit Patient Questionnaire.
  5. Completed Credit Policies form.

Prior to the initial visit, all forms should be completed. Upon arrival at Twin Cities Spine Center personal, (address, phone number, etc.) and insurance information will be verified. The consent for treatment (a part of the blue Credit Policies form) must be signed. Once registration is completed, patients will be called back to the first available examination room.

Once patients are called to an examination room, they will be prepared for the examination. A Polaroid photograph will be taken and kept in each patient's chart. In order for the physician to perform a physical evaluation, patients will be asked to change into a gown and bathrobe. A height, weight and blood pressure will also be obtained.

Once these preparations are complete, a member of the physician's team will review medical history and records and perform a physical examination. Team members may include a nurse clinician, physicians assistant, orthopaedic fellow or resident. Once this information is gathered it will be presented to the Twin Cities Spine Center physician. This physician may also discuss medical history, review records and perform a physical examination.

After the review of records and examination are complete, each patient will receive an explanation of diagnosis and recommendations for ongoing care which may include non-operative and operative treatments . Additional diagnostic testing (MRI, CT, etc.) may be ordered before final recommendations can be made.

At the completion of the visit clinic staff will provide instructions for follow up procedures or appointments; and give instructions on how to obtain additional information or follow up on any questions or concerns.

The time necessary to complete an initial visit is at least two hours. The complexity of patients' needs or emergency situations may necessitate longer appointments or additions to the clinic schedule. We apologize in advance for any delays these changes may make in scheduled appointments.
