Pre-Approval

Most insurance plans require pre-approval for specialty care. It is the patient's responsibility to obtain pre-approval from their insurance carrier. Certain insurance carriers also may require a specific referral from the patient's primary care physician.

Twin Cities Spine Center (TCSC) will assist patients in arranging all required authorizations needed for surgical procedures.Preferred Providers Twin Cities Spine Center is a provider for the following insurance plans*:

Twin Cities Spine Center will submit claims for all types of insurance (including worker's compensation, auto and liability) as a courtesy. Current insurance cards must be presented at each visit.

Worker's Compensation

Twin Cities Spine Center participates with most state Worker's Compensation programs. However, you may be responsible for portions of your bill not paid by Worker's Compensation.

Authorization from your Worker's Compensation carrier may be required before scheduling a surgery. TCSC will file a claim for authorized services. To expedite this procedure, you must have the adjuster's name, phone number and your claim number available at the time of your appointment.

Auto Insurance

TCSC will submit claims to auto insurance carriers but you will be responsible for portions of your bill not paid by your auto carrier. To expedite this procedure, you must have the adjuster's name, phone number and your claim number available at the time of your appointment.

Copayments

A copayment is an amount established by individual insurance plans and usually is specified on the insurance card. It is required that this amount be paid at the time of service.

Personal Balances

Any portion of your bill that is not covered by your insurance carrier becomes your personal responsibility. Twin Cities Spine Center will bill individuals on a monthly basis and personal balances are due upon receipt of the statement. Payment terms may be arranged through the Business Office.

For your convenience, Twin Cities Spine Center accepts cash, personal or cashier checks and MasterCard and Visa credit cards. There will be a $20 service fee on all returned checks.

Release of Information Authorization

A Release of Information Authorization is included in your registration materials. This form authorizes Twin Cities Spine Center to release information to your insurance provider as well as any health care provider involved with your current treatment. A new Release of Information Authorization should be completed any time your coverage changes and yearly thereafter to assure that your claims will be promptly filed and paid.
